Lakeshirts believes that each and every employee plays an important role in our success and that is why we strive to provide you with a benefits program that rewards you for the hard work and dedication you put forth every day.

The comprehensive benefits program that Lakeshirts offers you and your family is an important component of your total compensation package. A great deal of time and effort has been invested in designing, funding, and maintaining a quality benefits plan for you and your family.

Use this guide to obtain the information you need to make informed benefit choices for yourself and your family. The plans you choose to elect now will continue throughout the rest of the plan year unless you experience a qualifying life event change.

Eligibility

If you are a regular active full-time employee of Lakeshirts, and work 30 hours or more per week, you are eligible for the company benefits. You are eligible to participate on the first day of the month following 60 days of employment.

In addition, the following family members are eligible for certain benefits as well:

  • Legally married spouse
  • Dependent child(ren) up to age 26
  • Dependent Disabled child(ren)
  • Domestic Partner

Qualifying Events

You can change your benefit election during the year if you have a qualifying life event. Qualifying life events include, but are not limited to:

  • Marriage, divorce or legal separation.
  • Birth or adoption of a child.
  • Change in child’s dependent status.
  • Death of a spouse, child or other qualified dependent.
  • Change in employment status or a change in coverage under another employer-sponsored plan.

If you have a Qualifying Life Event and want to request a mid-year change, you must notify Human Resources and complete your election changes within 30 days following the event. Be prepared to provide documentation to support the Qualifying Life Event.

Medical Insurance with UnitedHealthcare

New Medical Carrier in 2025!

Lakeshirts understands the importance of medical coverage and is committed to providing high quality health care benefits to you and your family. We have a new Medical Carrier in 2025 and will now partner with UnitedHealthcare to offer more nationwide in-network providers.

The Surest D6500 Plan offers you a plan with no deductible, with copayments for all medical services and prescription drugs.

The $5,000 HSA Plan has lower premiums and a higher deductible. You can pair this plan with an HSA to help cover your eligible expenses throughout the year.

All plan options available provide 100% coverage for preventive care when received from a network provider.

Additional Services through UHC:

UHC Rewards: Your health plan now comes with a new way to earn up to extra money in your pocket! UHC Rewards is included in your health plan at no additional cost and is available to all members and their spouses on the Lakeshirts UHC plan. With UHC Rewards, a variety of actions, including many things you may already be doing, lead to rewards.

UHC Website and Mobile App: Download the UnitedHealthcare app today or visit MyUHC.com to monitor your health online! You can sign-in to gain access to your medical claims and documents, view your bills, track your UHC rewards, find in-network providers, and much more.

Dental Insurance with UnitedHealthcare

New Dental Carrier in 2025!

Lakeshirts offers dental coverage through plans administered by UnitedHealthcare. This is a new carrier for 2025. The plans encourage you to maintain good dental health by paying 100% of the cost for preventive care such as routine check-ups and cleanings. Please refer to your dental benefit summary available for an entire list of covered services, limitations and frequencies.

Vision Plans with EyeMed

Vision insurance can help you maintain your vision as well as detect various health problems. Lakeshirts’ voluntary vision insurance through EyeMed provides specified dollar amounts or discounts for the purchase of eyeglasses and contact lenses when you utilize an in-network provider.



 

Finding a Provider: With EyeMed you have access to one of the nation’s largest provider networks, consisting of optometrists and ophthalmologists along with national and regional optical chain locations. When you visit an in-network vision care provider, benefits are greater and there are no claim forms to file.

Life and AD&D Plans with New York Life

Beneficiaries: Don’t forget to put your loved ones first—add or update your life insurance beneficiary in Ease. Your beneficiary can be one or several people, and the same or different designation for all your benefits.  


Basic Life and AD&D Insurance

Lakeshirts provides eligible employees $30,000 of group life and accidental death and dismemberment (AD&D) insurance. Lakeshirts pays for the full cost of this benefit—meaning you are not responsible for paying any monthly premiums.


Voluntary Life and AD&D Insurance

In addition to the employer paid life insurance, employees may elect supplemental coverage for themselves, their spouse and/or children through Principal. When you enroll yourself and/or eligible dependents in this benefit, you pay the full cost of coverage through post-tax payroll deductions. You must elect Voluntary Life /AD&D coverage for yourself in order to cover your spouse and/or children.

 

Evidence of Insurability

If you do not elect Voluntary Life/AD&D coverage when you are first eligible and decide to enroll in coverage at a later date, evidence of insurability may apply. Evidence of insurability is a form that you will complete to verify your health status for coverage to begin.

You must provide EOI when you choose not to buy Voluntary Life for yourself or your dependents at this initial open enrollment then decide to add coverage later (such as Open Enrollment next year). 


Additional Information

Reduction Schedule: Life insurance amounts are reduced to 65% of your original benefit at age 65, and 50% of your original amount when you reach age 70.

Voluntary Benefits with New York Life - New in 2025!

You have the opportunity to purchase additional voluntary benefits through New York Life. You pay the full cost of this benefit through payroll deductions. Below are the benefit options available to employees. You are able to see the cost of these benefits along with additional details on the enrollment portal.

Accident Insurance

Accident insurance works to complement your medical coverage - and pays in addition to what your medical plan may or may not cover. It’s coverage that provides a financial cushion for life’s unexpected events by providing you with a lump-sum payment (one convenient payment all at once) when your family needs it most. The payment you receive is yours to spend however you like. Coverage includes payments for emergency treatment, hospitalizations, as well as treatment for broken bones, lacerations, and burns. 

Critical Illness Insurance

Many illness related expenses are out-of-pocket expenses such as co-insurance, deductibles, and indirect expenses that major medical plans are not designed to cover. New York Life's critical illness plan coverage helps offset these costs. This plan also includes benefits for transportation & lodging, surgical procedures, extended care benefits that include family care, skilled nursing and home health care/hospice and more.

Short and Long-Term Disability Plans with New York Life

Lakeshirts provides employees with the opportunity to elect short-term and long-term disability income benefits. Without disability coverage, you and your family may struggle to get by if you miss work due to an injury or illness. In the event you become disabled from a non-work-related injury or illness, disability income benefits will provide a partial replacement of lost income. Please note that you are not eligible to receive disability income benefits if you are receiving workers’ compensation benefits.

Retirement Plan through John Hancock

Building a strong financial foundation for your future is an ongoing process. Lakeshirts, Inc. & Affiliates 401(k) Profit Sharing Plan, managed by John Hancock, is designed to help you save for retirement by offering you tax advantages as well as the convenience of payroll deductions.

All full-time and part-time employees over age 18 are eligible to participate in the 401(k) Profit Sharing Plan after one full year of service. A full year of service is defined as at least 1000 hours during a 12-month period. 

Contributions

You are able to contribute 0-100% of your total pay on a pre-tax basis. Your total salary deferral may not be more than $23,500. If you are 50 years old or older during the plan year and you have met the annual IRS Deferral limit, you may contribute a catch-up deferral of up to $7,500.

Employer Match

Lakeshirts Inc. will make a discretionary match to all eligible participants. Lakeshirts Inc. intends to make a matching contribution of 50% of employee deferrals up to 4%.

Vesting

Employees are always 100% vested in their own contributions.

Employer contributions are vested based upon the following schedule:

1 Year of Service: 20%

2 Years of Service: 40%

3 Years of Service: 60%

4 Years of Service: 80%

5+ Years of Service: 100%

 

Distributions

In addition to lump sum distributions after separation from services, In-Service Withdrawals are available after reaching age 59 ½. Withdrawals of certain money types may be available due to Financial Hardship.
